RESUMO JUSTIFICATIVA E OBJETIVOS: Dentre as classificações das disfunções temporomandibulares, as dores miofasciais nos músculos da mastigação são as mais frequentes. Sua etiologia multifatorial dificulta o tratamento. Reconhecer outros locais de dor relacionados à disfunção temporomandibular pode ajudar no controle das comorbidades e consequentemente melhorar o seu quadro. O objetivo deste estudo foi avaliar a presença de dor no corpo em pacientes com disfunção temporomandibular, a frequência desses relatos e sua localização. MÉTODOS: Foram avaliados 328 prontuários do Centro de Pesquisa Odontológica São Leopoldo Mandic, com diagnóstico de disfunção temporomandibular muscular. Os pacientes foram avaliados por meio de desenhos de mapa corporal para determinar a localização de queixas de dor. RESULTADOS: Dos 328 prontuários analisados, 180 (55%) apresentaram registro de dor pelo corpo (160 mulheres, 20 homens), e 148 (45%) apresentavam dor apenas na região da face (116 mulheres, 32 homens). As áreas com maior relato de dor foram: cervical, lombar e ombros. O gênero feminino apresentou maior frequência de dores no corpo (com dor n=160, sem dor n=116, p<0,001) que o gênero masculino (com dor n=20 e sem dor n=32) com diferença estatística. Na maior parte dos casos a dor acometeu os dois lados do corpo (face 67% bilateral e corpo 92% bilateral) CONCLUSÃO: A maioria dos pacientes com diagnóstico de disfunção temporomandibular apresentou dor em outras partes do corpo além da face. A região anotada nos desenhos do corpo humano com maior acometimento da dor foi a cervical seguida da lombar e ombros.

ABSTRACT BACKGROUND AND OBJECTIVES: Among temporomandibular disorder classifications, masticatory muscles myofascial pain is the most frequent. Its multifactorial etiology makes its treatment difficult. Identifying other painful sites related to temporomandibular disorders may help controlling comorbidities and, as a consequence, improving their symptoms. This study aimed at evaluating the presence of body pain in temporomandibular location. METHODS: We have evaluated 328 medical charts of the Dental Research Center São Leopoldo Mandic, with diagnosis of muscular temporomandibular disorder. Patients were evaluated by means of a body map to locate pain complaints. RESULTS: From 328 analyzed medical charts, 180 (55%) had body pain (160 females, 20 males), and 148 (45%) had facial pain only (116 females, 32 males). Areas with most frequent pain reports were cervical, lumbar and shoulders.Females had more body pain (with pain n=160, without pain n=116, p≤0.001) as compared to males (with pain n=20, without pain n=32) with statistical difference.In most cases pain has affected both body sides (bilateral face 67%, bilateral body 92%). CONCLUSION: Most patients with temporomandibular disorder had pain in body parts different from the face. Regions marked in human body drawings with more pain were cervical followed by lumbar and shoulders.

The objective was to investigate the prevalence of temporomandibular dysfunction TMD - in severe and moderate hemophiliac A and B patients and healthy men as control group. Hemophilia complication is chronic arthropathy that results from repeated joint bleeding, leading to limited movement. Limitation of jaw movement is present in patients with TMD. Hemophiliac patients were recruited in the Hemophilia outpatient clinic at Escola Paulista de Medicina, Universidade Federal de São Paulo (UNIFESP). The control group was composed of voluntary subjects recruited among medical and dental students of UNIFESP. Both groups were screened for TMD symptoms according to the European Academy of Craniomandibular Disorders questionnaire. The Research Diagnostic Criteria further evaluated those considered positive for TMD. The results showed a similar prevalence of TMD in the hemophiliac group compared to the control group (n= 38, n= 79; p= 0.7). There were no significant differences in severity of sign and symptoms between the groups. In conclusion, patients with hemophilia do not have a higher prevalence of temporomandibular disorders, indicating absence of hemorrhage in temporomandibular joint.

El objetivo fue investigar la prevalencia de trastornos temporomandibulares (TTM) entre pacientes hemofílicos A y B severos y moderados, y hombres sanos como grupo de control. Una complicación de la Hemofilia es la artropatía crónica como resultado de una hemorragia articular a repetición, limitando el movimiento en el tiempo. La limitación del movimiento de la mandíbula está presente en pacientes con TTM. Los pacientes hemofílicos fueron reclutados en la clínica de atención ambulatoria de hemofilia en Escola Paulista de Medicina, Universidade Federal de São Paulo (UNIFESP). El grupo control estaba compuesto por sujetos voluntarios reclutados entre los estudiantes de Medicina y Odontología de UNIFESP. Ambos grupos fueron evaluados por síntomas de TTM según cuestionario de trastornos craneomandibulares de la Academia Europea. Los criterios de diagnósticos de investigación evaluados se consideraron positivos para TTM. Los resultados mostraron una prevalencia similar de TTM en el grupo de hemofílicos en comparación con el grupo control (n= 38, n= 79; p= 0,7). No se encontraron diferencias significativas en la gravedad de los signos y síntomas entre los grupos. En conclusión, los pacientes con hemofilia no tienen una mayor prevalencia de trastornos temporomandibulares, indicando la ausencia de hemorragia en la articulación temporomandibular.

The jaw muscles are essential components in the stomatognatic system. Their complex architecture allows them to execute several motor tasks. One of the structural peculiarities is the presence of hybrid and neonatal fibers.We studied the differences of the fiber-type in masseter and temporalis muscles along the first to nineth decades in both genders. Seventy-four (74) samples were analyzed by immunohistochemistry. Slow and fast muscle fibers distribution was similar in both muscles in both genders. Hybrid fiber was observed in all decades, and its frequency decreased significantly (p<0.001) with aging in masseter. Neonatal myosin expression was observed in all decades, its expression was more frequent in masseter (p=0.01), and males in temporalis (p=0.025). Decrease of the cross sectional area of fast and slow fibers, and decrease of capillary density were detected with aging. These morpho-immunohistochemical alterations on masseter and temporalis muscles correlated to the decrease in bite force with aging.

Los músculos cráneomandibulares son componentes esenciales en el sistema estomatognático. Su arquitectura compleja les permite ejecutar variadas tareas motoras. Una de sus características estructurales es la presencia de fibras musculares híbridas y neonatales. Se estudiaron las diferencias del tipo de fibra en los músculos masetero y temporal en la primera a novena décadas dela vida en ambos sexos. Setenta y cuatro (74) muestras se analizaron por inmunohistoquímica. La distribución de lãs fibras musculares lentas y rápidas fue similar en ambos músculos en ambos sexos. Fibras musculares híbridas fueron observadas en todas las décadas, pero su frecuencia disminuyó significativamente (p <0,001) con el envejecimiento en el masétero. La expresión de miosina neonatal se observo em todas los grupos analizados, aunque su expresión era más frecuente en los músculos maseteros (p = 0,01) y en varones en el temporal (p = 0,025). Se observo una disminución del área de la sección transversal de las fibras rápidas y lentas, y disminución de la densidad capilar al aumentar La edad. Estas alteraciones morfológicas en los músculos masetero y temporal se correlacionan con la disminución de la fuerza asociada al envejecimiento.

The aim was to analyze the characteristics of the temporomandibular joint (TMJ) in a group of adult patients suffering from Steinert's muscular dystrophy (DM1). This study included 42 adult patients aged between 21 and 69 years (mean = 38.7619; SD = 12.74) who were diagnosed for DM1. Study was conducted using the MRI of right and left TMJ sagittal images taken in maximum intercuspidation position and maximum oral opening without pain, and the following were discussed: a) the quality of the cortical bone in the mandibular fossa, tuberosity, and mandibular head; b) the relationship of mandibular headdiscjoint tuberosity in maximum intercuspidation position; c) the anatomical shape of the articular disc. All patients showed abnormalities in the shape and surface of the cortical bone in the mandibularfossa, tuberosity, and the mandibular head. With regard to the relationship of the mandibular head in the mandibular fossa, 41% was found in the region 2B, 29% in 1B, 18% in 1A, 9% in 2C, and 3% in 2A of the TMJ. About 49% of the disc in maximum intercuspidation position was found on the mandibular head, 26% were anterior displaced, 14% had anterior dislocations, 38% had alterations in the form of disc, and 30% had preserved the anatomical shape. However, 98% of the discs showed hyposignal in T1 and T2. Using MRI, the decrease in the muscle activity in patients with DM1 was found to generate degenerative changes visible in the TMJ.

El objetivo de este estudio, fue analizar las características de la articulación temporomandibular (ATM) en un grupo de pacientes adultos portadores de distrofia muscular de Steinert (DM1). Fueron evaluados 42 pacientes adultos, con edades entre 21 y 69 años (Media=38,7619; DE=12,74) diagnosticados con DM1. Se realizó un estudio por RNM de ATM mediante imágenes sagitales de ATM derecha e izquierda en posición de máxima intercuspidación (MIC) y apertura máxima sin dolor, y se analizaron a) la calidad de las corticales de la fosa mandibular, tuberosidad y cabeza mandibular, b) la relación cabeza mandibular- disco- tuberosidad articular en MIC, c) la forma anatómica del disco articular. Todos los pacientes presentaron alteraciones en la forma y superficie de las corticales de la fosa mandibular, tuberosidad articular y cabeza mandibular. Con respecto a la relación de la CM en la FM, el 41% se encontró en la región 2B, el 29% 1B, 18% 1A, 9% 2A y 3% 2C de la ATM. El 49% de los discos en MIC se encontraron sobre la cabeza mandibular, 26% estaban desplazados anteriormente, 14% presentaron un dislocamiento anterior. El 38% presentó alteraciones de la forma del disco y 30% presentaron conservada la forma anatómica. El 98% de los discos presentaron hiposeñal en T1 y T2. La disminución de la actividad muscular en los pacientes con DMS genera cambios degenerativos en la ATM visibles con RNM.

The study objective was to evaluate the prevalence of referred dental pain (RDP) in a group of Brazilians subjects and identify possible partnerships with sex, age and the presence of periodontal or periapical lesions. A descriptive cross-sectional study was designed, 98 patients between 14 and 64 years old (59 women and 39 men), who consulted by dental pain were evaluated clinically and radiographically in order to determine the cause and partnership with periapical and periodontal lesions and its possible territories projection other than their origin. The prevalence of RDP was 31.6%, higher in women (67.74%) though without statistical significance. The RDP was presented at a 45.16% together with periapical lesion and a 25.8% along with periodontal lesion. There was no relationship between age and RDP presence. The high prevalence of RDP found reinforces the need for a diagnosis of orofacial pain.

El objetivo de este estudio fue analizar la prevalencia de dolor referido dental (DRD) en un grupo de sujetos brasileros y determinar las posibles asociaciones con sexo, edad y la presencia de lesión periapical o periodontal. Se diseñó un estudio descriptivo de corte transversal, con 98 pacientes, de entre 14 y 64 años (59 mujeres y 39 hombres), que consultaron por dolor dental, ellos fueron evaluados clínica y radiográficamente con el fin de determinar la causa y la asociación con lesión periapical y periodontal y su posible proyección a territorios distintos de su origen. La prevalencia del DRD fue de 31,6%, mayor en mujeres (67,74%) aunque sin significancia estadística. El DRD se presentó en un 45,16% junto con lesión periapical y un 25,8% junto a lesión periodontal. No se encontró asociación entre la edad y la presencia de DRD. La alta prevalencia de DRD encontrada refuerza la necesidad de un diagnóstico etiológico del dolor orofacial.

ABSTRACT The Steinert's muscular dystrophy type 1 involves frequently the masticatory muscle causing muscular weakness and orofacial alterations; this entity is characterized by to present the anticipatory phenomenon where la offspring of a involved person develops the disease in an early way. The aim of present paper is to report 2 cases in mother and daughter diagnosed with the DM1 and to analyze the oral clinical features and the effects of anticipatory phenomenon of disease by stomatologic examination, measurement of masticatory strength and magnetic nuclear resonance of temporomandibular joint. The clinical expression of this disease was earlier in the daughter than in the mother, which was related to a greater number of lost teeth and of degenerative changes en la TMJ associated with a lesser masticatory strength.

La distrofia muscular de Steinert tipo 1 afecta frecuentemente a los músculos de la masticación y puede causar debilidad muscular y alteraciones bucofaciales. Esta enfermedad se caracteriza por presentar el fenómeno de anticipación, en el cual la descendencia de una persona afectada desarrolla la enfermedad de manera más precoz. El objetivo de este trabajo fue reportar 2 casos de una madre y su hija con diagnóstico de distrofia muscular de Steinert. Se realizó un análisis de las características clínicas bucales y los efectos del fenómeno de anticipación de esta enfermedad mediante el examen odontológico, la medición de la fuerza masticatoria y la resonancia magnética nuclear de la articulación temporomandibular. La expresión clínica de la enfermedad fue más precoz en la hija que en la madre, lo que se relacionó con un mayor número de estructuras dentarias perdidas y de cambios degenerativos en la articulación temporomandibular, asociados a una menor fuerza masticatoria.

The objective of this study was to assess the association between an increase in the proportion of the superior head of the lateral pterygoid muscle (SHLP) inserted into the temporomandibular joint (TMJ) disc and the presence of anterior disc displacement diagnosed by using MRI. We analyzed 75 images of patients with anterior displacement and 75 controls with normal relationship between the mandibular head and the articular tubercle also diagnosed by MRI. Levels of insertion were measured. Descriptive and analytic statistics were calculated. Analyses of differences were made by Fischer's exact test and also were calculated clinical significance (OR, CI = 95%). The average insertion of SHLP was 71.14% (SD 14.13) in controls and 67.80% (SD.16.23) in cases group. The only relationship that presented a significant association (OR 5.61) was found when comparing subjects with a percentage of inserting the disc into the SHLP at 50% with those who had higher levels of insertion (50%). Our results suggest that the highest percentage of insertion serves as a protective factor for anterior disc displacement, probably due to greater control of neuromuscular activity.

El objetivo de este estudio fue evaluar la asociación entre el aumento de la proporción de la cabeza superior del músculo pterigoideo lateral (CSPT) insertado en la articulación temporomandibular (ATM) y la presencia de desplazamiento discal anterior diagnosticado mediante resonancia magnética. Se analizaron 75 imágenes de pacientes con desplazamiento discal anterior y 75 controles con una relación normal entre la cabeza de la mandíbula y el tubérculo articular, también diagnosticada por resonancia magnética. Los niveles de inserción fueron medidos. La estadística descriptiva y analítica fueron calculadas. Los análisis de las diferencias se hicieron por la prueba exacta de Fischer, y también se calculó la significancia clínica (OR, IC=95%). La inserción media de la CSPT fue 71,14% (DS 14,13) en los controles y 67,80% (SD 16,23) en los casos de grupo. La única relación que presentó una asociación significativa (OR 5,61) se encontró al comparar los sujetos con un porcentaje de inserción en la CSPT 50% con aquellos que tenían niveles más altos de inserción (50%). Nuestros resultados sugieren que el mayor porcentaje de inserción sirve como un factor de protección para el desplazamiento discal anterior, probablemente debido a un mayor control de la actividad neuromuscular.

The constitution and shape of superior head of the lateral pterygoid muscle (SHLP) inserts remains a topic of interest in the literature. The purpose of this study was to analyze by magnetic resonance imaging (MRI) the temporomandibular joint (TMJ) of individuals without signs of temporomandibular dysfunction. One hundred seventy-eight MRI images of one hundred and three patients were utilized, and the surface and percentage of the SHLP insert into the articular disc and condilar process was determined. In women, the average insertion into the disc was found to be between 5.7-5.5mm (SD 1.5-1.3), corresponding to 69.8-70.7% of the SHLP. In men, the average insertion into the disc was between 6.2-6.12mm (SD 1.8-1.9), or 68.9-74.2%. There were no differences in terms of the sex of the patients or the side of the joint. We found a significant percentage of SHLP fibers embedded in the disc of asymptomatic TMJs, and its role in the anterior disc displacement and the development of the temporomandibular dysfunction should be revised.

La constitución y la forma de inserción de la cabeza superior del músculo pterigoideo lateral (CSMPL) sigue siendo un tema de interés en la literatura. El objetivo de este estudio fue analizar mediante resonancia nuclear magnética (RNM) la articulación temporomandibular (ATM) de individuos sin signos de disfunción temporomandibular. Fueron utilizadas 178 imágenes de RNM pertenecientes a 103 pacientes. Se determinó la superficie y porcentaje de la inserción de la CSMPL en el disco y proceso condilar. En mujeres, la media de inserción en el disco estuvo entre 5,7-5,5 mm (SD 1,51,3) correspondiente al 69,8-70,7% de la CSMPL. En hombres, la media de inserción en el disco estuvo entre 6,2-6,12mm (SD 1,8-1,9) correspondiente al 68,9-74,2% de la CSMPL. No hubo diferencias en cuanto al sexo de los pacientes o lado de la articulación. Hemos encontrado un porcentaje significativo de las fibras de la CSMPL insertadas en el disco de ATMs asintomáticas, y su papel en el desplazamiento anterior del disco y el desarrollo de disfunción temporomandibular debe ser revisado.

The lateral pterygoid muscle (LPM) is important in the physiology and dysfunction of the temporomandibular joint (TMJ), which has described the presence of a third head in the lateral pterygoid muscle (THLPM). The purpose of this study was to evaluate the prevalence of THLPM and its relationship with the TMJ disc through magnetic resonance imaging (MRI). It was evaluated 178 MRI of the TMJ of healthy individuals, in images that presented THLPM, and these were defined and measured at the surface inserted into the disc. The prevalence of THLPM was 20.22% (n = 36). Insertion of THLPM was made entirely on the TMJ disc, the average area of insertion was 4.14mm (SD 1.35) in women and 4.67 (SD 2.68) in men, but these differences were not significant. We concluded that the LPM has a complex structure, and that a THLPM could contribute in the development of TMJ disc function alterations and anterior disc displacement.

El músculo pterigoideo lateral (MPL) tiene importancia en la fisiología y en la disfunción de la articulación temporomandibular (ATM), se ha descrito la presencia de una tercera cabeza en el músculo pterigoideo lateral (TCPL). El propósito de este estudio fue evaluar mediante resonancia nuclear magnética (RNM) la prevalencia de la TCPL y su relación con el disco de la ATM. Se evaluaron 178 RNM de ATM de individuos sanos. En las imágenes que presentaron TCPL éstos fueron delimitados y se midió la superficie inserta en el disco. La prevalencia de la TCPL fue del 20,22% (n=36). La inserción de la TCPL se realizó completamente en el disco de la ATM, la media del área de inserción fue de 4,14mm (DS. 1,35) en mujeres y 4,67 (DS. 2,68) en hombres, estas diferencias no resultaron significativas. Concluimos que el MPL presenta una estructura compleja y que una TCPL podría participar en el desarrollo de alteraciones en la función discal y en la presencia de desplazamiento anterior del disco.

The purpose of this study was to analyze the condylar position in the maximum mouth opening in a group of individuals with and without temporomandibular joint disorders (TMD) using corrected magnetic resonance imaging (MRI). It was analyzed the MRIs of 200 temporomandibular joint (TMJ) of 100 adults (25 males and 75 females). The images protocol of the MRIs were in corrected sagittal plane T1, with the patient kept at a maximum mouth opening without pain. All the images were obtained parallel to the Frankfurt plane. The images were split by two lines parallel to the upper base of the image which was obtained parallel to the Frankfurt plane, tangential to the articular tubercle and the condyle of each TMJ, and set along the discrepancy of the condylar position on both sides and the position with regard to the articular tubercle. The discrepancy was 45% overall, the most common condylar position types were 5 and 6 on the right and left sides in both the male and female TMJ patients. In this regard, the highest mean values were found for the maximum mouth opening. Our results support the hypothesis that the condylar position is not directly related to the magnitude of the oral opening.

El propósito de este estudio fue analizar mediante resonancia nuclear magnética (RNM) la posición condilar en apertura máxima en pacientes sin alteraciones de la articulación temporomandibular (ATM). Se utilizaron 200 imágenes de 100 individuos adultos (25 hombres y 75 mujeres) observadas en plano sagital corregido en T1 con el paciente en apertura máxima sin dolor, la región de la ATM se dividió mediante dos líneas perpendiculares entre sí, tangentes a la cabeza mandibular y al tubérculo articular y se estableció la discrepancia de la posición condilar en ambos lados y la posición condilar con respecto al tubérculo articular. La discrepancia general fue del 45%, las posiciones condilares más frecuentes fueron los tipos 5 y 6 en los lados derecho e izquierdo en ATM de hombres y de mujeres, en esta relación se encontraron los mayores valores medios de apertura oral máxima. Nuestros resultados apoyan la hipótesis que la posición condilar no se encuentra en relación directa con la magnitud de la apertura oral.
